Bernards Field Hockey

Below you will find REQUIRED forms for participation on the Bernards Field Hockey Team. 

Please note that all of the forms must be handed in and approved before an athlete can participate.  Therefore, ALL FORMS ARE DUE NO LATER THAN JULY 15th IN THE MAIN OFFICE OF THE HIGH SCHOOL!
